Required Parent Consent Form for Counselor- and Health-Related Services - SB 12
Texas Senate Bill 12 (SB 12) now requires schools, including Ingram ISD, to obtain written parental consent before providing students with physical or mental health services. This includes services that may be offered by school nurses, counselors or other support staff.
For your child to receive health and support services while at school, you must complete the linked SB 12 Parent Consent Form.
